How to use this document
Copy the template below into your quality system. Replace every <<FILL: ...>> placeholder with your company, system, and process specifics, then route it through your own document control, review, and approval. The filled specimen shows one worked example; adapt the numbers and scope to your operation and verify each cited regulation or standard against the current source before you rely on it.
This is a generic, originally worded starting point. It contains no confidential, proprietary, or employer material, it is drafted with AI assistance and reviewed, it is not legal or compliance advice, and adopting it does not by itself create or prove compliance. See the Disclaimer and Terms.
This is a ready-to-use traceability matrix for a Continued Process Verification program. Its job is narrow and specific: put every monitored parameter, and every parameter deliberately left out, in one table that traces back to the Stage 1 risk assessment, so a reviewer can check the CPV plan’s coverage in a single pass instead of cross-referencing two documents by hand. This matrix is a companion to the CPV plan, not a replacement for it; the plan states the program’s methods and escalation rules, this matrix proves each parameter’s inclusion or exclusion is traceable. Replace every <<FILL: ...>> placeholder with your own specifics. A filled specimen follows.
1. Purpose
This matrix traces every parameter monitored under the CPV plan referenced above back to the Stage 1 criticality assessment that identified it, states the monitoring method and control limit basis for each, and records why any parameter the Stage 1 assessment flagged as a CQA or CPP is not separately charted, if that is the case. A monitored parameter with no traceable source, or a known CPP absent from this matrix with no documented reason, is the specific gap this document is built to prevent.
2. Scope
3. Parameter traceability
Add one row per parameter. Do not compress multiple distinct CQAs or CPPs into one row for brevity; a reviewer checking this matrix against the Stage 1 document needs a one-to-one match.
4. Parameters identified in Stage 1 but not independently charted
Leave this section explicitly empty, stated as “none,” rather than omitting it, if every Stage 1 CQA and CPP is charted above. An inspector comparing this matrix to the Stage 1 assessment is checking for exactly this kind of silent gap.
5. Multivariate monitoring, if applicable
6. Revision history
7. Approvals
Filled specimen
Extract for an example allogeneic cell therapy drug substance process. Company and numbers are illustrative.
Section 4, parameters not independently charted:
Section 5: no multivariate model currently in use; fold expansion and final viability are reviewed for co-movement manually each cycle, and a multivariate model is planned once the commercial batch count passes 40.
Common inspection findings this matrix prevents
- A known CPP from the Stage 1 risk assessment with no corresponding chart anywhere in the CPV documentation, discovered only when an inspector cross-references the two documents directly.
- A parameter charted in the CPV plan with no stated Stage 1 source, so its inclusion looks arbitrary rather than risk-based.
- Provisional or interim control limit bases left unstated, so a reviewer cannot tell a mature, established limit from a limit seeded from four PPQ batches.
- Parameters correlated with a charted CQA left completely unaddressed, with no statement of where that risk is covered.
How to adapt this matrix
- Build this matrix directly from the Stage 1 risk assessment’s CQA and CPP list, not from whatever the historian currently trends; add rows for gaps the current program has not yet closed rather than omitting them.
- State the control limit basis honestly in every row: established, provisional and seeded, or specification-as-interim-proxy. A reviewer needs to know which parameters carry mature statistical weight and which do not.
- Keep Section 4 current. A parameter that moves from “not separately charted” to “charted” needs a matrix update, not just a CPV plan update.
- Revisit this matrix at every CPV plan revision and at the annual review, and reconcile it against the current Stage 1 risk assessment if that document has itself been revised.
