How to use this document

Copy the template below into your quality system. Replace every <<FILL: ...>> placeholder with your company, system, and process specifics, then route it through your own document control, review, and approval. The filled specimen shows one worked example; adapt the numbers and scope to your operation and verify each cited regulation or standard against the current source before you rely on it.

This is a generic, originally worded starting point. It contains no confidential, proprietary, or employer material, it is drafted with AI assistance and reviewed, it is not legal or compliance advice, and adopting it does not by itself create or prove compliance. See the Disclaimer and Terms.

This is a ready-to-use handoff log for transferring person-in-plant coverage between shifts on a campaign that runs longer than one PIP can reasonably cover alone. Complete it at every shift change, whether by a brief overlap window or, where that is not practical, as a standalone log the incoming PIP reviews and signs before assuming coverage. Replace every <<FILL: ...>> placeholder. A filled specimen follows.

Instructions

Complete this log before the outgoing PIP leaves the floor or ends the call, never afterward from memory. The incoming PIP must review every field and sign confirming receipt before assuming coverage; an unsigned handoff is an open gap, not a completed one. Retain per <<FILL: document retention period>>, filed alongside the campaign’s visit reports.

R. Okafor’s shift opened by checking the electronic batch record for the equipment check documentation M. Chen had committed to; finding it not yet entered, R. Okafor prompted the CDMO’s night quality representative, who entered it before the 22:00 batch disposition review. Without this log, R. Okafor would have had no reason to check for a commitment made before the shift started.

Common inspection findings this log prevents

  • A verbal commitment made to one PIP that is never seen again because the next PIP on the account never knew it existed.
  • A multi-shift campaign with no documented record of how coverage was actually transferred between PIPs, leaving an invisible gap exactly at each shift change.
  • An open item from one shift silently absent from the next shift’s attention, discovered only when it recurs weeks later.

How to adapt this log

  1. Set your own handoff method, overlap window, call, or log-only, as the standard for each campaign in its oversight plan, not improvised at the first shift change.
  2. Never mark a handoff complete without the incoming PIP’s signature; an unsigned entry is an unconfirmed transfer of coverage.
  3. Carry forward any unresolved open item or unlogged commitment to the next handoff entry until it is actually closed, not just until the current shift ends.
